Peripheral artery disease (PAD) screening by ankle brachia! index (ABI) though part of recommendations  is not commonly performed.  Local  studies  on PAD with  diabetes  has no risk factor  analysis   while  none  is  currently available  on  pre• diabetics.  This study will  help promote AB!  in the clinics and risk  factor  determination   in candidates  for  further  vascular work-up,  particularly    in the  Philippines   where  healthcare expenses are not government-subsidized.


Objective:   To determine  the prevalence  and risk factors of  peripheral  arterial    disease    using  ABI  among  Filipino prediabetic    and  diabetic   patients   seen  at  the  out-patient department.


Methods: A cross-sectional   study on prevalence  and  risk factors  of PAD among pre-DM  and DM Filipinos  aged ~ 18 years with no previous cardiovascular  events, seen outpatient at  St.  Luke's   Medical   Center,   Quezon  City  from  July  to December   2016.    Demographic   data  and  results   of serum creatinine,   lipid profile,  and glycohemoglobin   from patients' charts were gathered.  Loss of protective  sensation (peripheral neuropathy)   was tested  using  mouofilament test.  PAD was determined    by ABL   using   a  hand-held      Doppler   device. Measures  of central   tendency   (mean)   and  percentages   were used for appropriate  data. Comparisons  of means  were done using independent  T-tests. Multiple  logistic  regression analysis was used to assess  the association   of risk factors  with  PAD (ABI  <0.91).


Results:  In  total,  166  subjects were enrolled (100  DM, 66 pre-DM).  The prevalence  of PAD among diabetics  was 19%, while   none among   pre-DM.  PAD subjects  had higher  SBP (p=0.015), total cholesterol (p=0.032),  triglyceride  levels (p=0.003),  and glycohemoglobin   (p=0.004) than the non-PAD group.  Additionally,    they  had  a longer  duration   of  DM (p=0.0001)   and lower  estimated   GFR (p<0.0001).


Conclusions: Screening  for PAD using  ABI among diabetics with concomitant hypertension,  elevated HbAlc,  and dyslipidemia   should   be conducted  even without  a prior cardiovascular     event.  No case of PAD using ABI in the prediabetes    group was detected.
